Insurance Information

Please remember to bring your insurance cards with you. If you have HMO coverage, an insurance referral from your primary care physician will be necessary. You can do this by contacting your primary care physician's referral secretary. Please be sure to secure an insurance referral prior to your visit. If a referral is not obtained, you will be asked to sign a waiver that states you will be financially responsible for your appointment or we may have to reschedule to another day. Please be advised that if your insurance has a co-payment, that co-payment will be collected on the day of your appointment. If your co-payment is not paid on the date of service, effective January 1, 2012, there will be a $10 administrative fee applied to your bill.
