Reports and X-ray Films
If you have had any x-rays, ultrasounds, CT scans, and/or MRI's/MRA's specific to your reason for visit , please be sure to notify our staff as to where and when you had them performed. We will ask that you obtain the films and the report for these x-rays and bring them with you to your appointment.

Questions for the Doctor
Please remember to write down any questions you may have for the doctor beforehand, so that you will not forget any of your concerns. However, if you have questions after your visit, please do not hesitate to call our office and someone from our staff will be happy to answer any questions or concerns you may have.

In consideration to those that have sensitive allergies, we ask that you be courteous to other patients and, if possible, do not wear any perfume or cologne on the day of your visit.
